Try opening the maps outside the game, in a text or hex editor. Both one of the included skirmish maps and yours with the problem. Might be able to spot differences and see what's wrong.

Can you open one of the included multiplayer maps? Perhaps examining that will help? One thing I found about those maps is they're not hard limited to the number of players listed. I found an older copy of the vstdlib.dll that mdldecompiler.exe (the supposedly bugfixed version of the one by CannonFodder) complained about. With that and tier0.dll in the same folder, it works. Valve updated vstdlib.dll (among other things) and that broke the decompiler, which nobody has bothered...

Yup, found that and VTF Explorer, also free and independent of Steam, for extracting textures from VTF files. What I haven't yet been able to find is an .MDL decompiler that doesn't require Steam. I have found plugins for 3D Studio and Blender to import the 3D data *after* it's extracted from the .MDL
